"The Elephant Man" directed by David Lynch. This tale of a pure soul struggling to be heard over the prejudice of the many is quite heart-rending. Perhaps Lynch's finest moment - Both disturbingly odd and surprisingly humane.

Not sure about saddest, but most depressing is Monster's Ball. I wanted to slit my wrists by about 20 minutes into it, and it didn't get better.
I'm a little surprised that others are saying "Life is Beautiful". While it was certainly tragic in one sense, I felt that the overall feeling was one of a "triumph of the spirit", so it actually left me somewhat uplifted. I cried like a baby, but found it more poignant than sad.

The saddest movie I've ever seen is Passion of the Christ.
I know what happened before i saw it but watching it made it all the more real.

That's tough. Maybe The Last Samurai or Braveheart? Memento was sad in a weird way. The Sixth Sense was sad, but the end was shocking and almost distracted from the sadder aspects.
