Go to the school district office as a start and ask. I think that should get you started.

You should go to the last known address, to begin your search! Take help from the school, if you can, to trace the great teacher, who may be in the address books of a few old students! I am sure you will find her soon, as a few old students may be in touch with her, and will be happy to help you! Good luck!

Contact your old school, via letter, and ask if any teachers or other staff members now working there remember her. If not, ask the Chamber of Commerce or an old bank, hospital, or other institution in the town if any elderly people remember her, and know where she went.
