this happened three times last night! I would be hard while fingering her then when I put the condom on it got too soft to insert it in her. What can I do next time that can prevent this?

Were you nervous? That might be why. Stage fright.
Are you not used to condoms? That could be it too.
Were you drunk? If you were drunk then that most likely was it.
Also if you were tired, or a combination of some of those, that could be it too.

Sounds like simple performance anxiety. Not that uncommon, especially when with a new partner. Try to relax a bit. Try to take some of the emphasis off of intercourse. Show her you can be the best lover she's ever had even without intercourse. The key is to concentrate on the sexual journey rather than putting all of your focus on the intercourse destination.
If that doesn't help, or help enough, then you could use a c**k ring which will help maintain the erection by restricting blood flow out of the penis. A shoe lace tied tightly around the base of the penis and scrotum while erect would do the same thing. Do try to avoid using anything that restricts blood flow for more than 20 minutes.
Remember, the more you can just relax and enjoy, the less this will be a problem for you.
