"Removed" refers to how many generations separate the people. For example, my "first cousin once removed" is my cousin's child, one generation away. My cousin's grandchildren would be my "first cousins twice removed" (or will be, when I'm old enough to have any!).

It means a generation off. For example, your first cousin would be an equal number of generations from the point of relation, but your cousin's kids would be a generation off so be your "first cousins once removed." Thus while you and your cousin would have the same grandparents, your grandparents would be the great grandparents of your first cousin once removed.
