I know just her first name, and license plate of her car. But I don't know where she lives or her last name.

How can I file a restraining order without any of this pertinent info?

NOTE: A Whole bunch of people in the 100s at church saw her when she was running after me and I told her to stop harrassing me and it was right in front of the priest also, and she laughed.

Give all the pertinent info you have -- and get the names of those witnesses too, so they can get witness statements -- the police. They will be able to locate this woman based on her DMV info from the plate number, and your priest or church might have it as well, if she is also a member.
But it's not your job to do the fact-finding if this woman is breaking the law, that's what cops are for. Give them all the information you have and let them sort it out.

Talk to a police officer, if you know her liscence plate number, there's her address and last name.
Explain to them what's going on and they'll help you with the information you need to file a restraining order.

If you have her liscence plate number, you can take it to the police, who can get you the information you need and get you a restraining order.
