Water temp and laundry soap can have a factor in the results of the stain removal. If it is blood then soak first in cold water then pretreat before washing. If it is sweat then soaking in a vinegar and water solution will help. All other stains use hot water and a prewash or bleach.

Try using bleach:if they are white--use chlorine bleach:if they are colored, use dry bleach. The chlorine bleach will work always better. The difference is that in the dry,it will not be a chlorine type.It will be a calcium type.
It is difficult to get them as clean as new.You can try,but you will find that the cloth will fall apart if you bleach too much.
BTW:it is not gross,it is an everyday thing.
Perhaps the easily offended on this page do not think about toilet paper and what it does.Maybe it injures their conscience.

I've never had a pair to come out completely clean, not even if I use bleach. Someone else may know of an answer.

If you have a continuing problem, then buy white cotton undies, then soak them first in a bucket with a cup of bleach and hot water.

(Just like your grandma or great grandma used to do with cloth diapers for babies.)

You can soak your undies any color in hot water with 1/2c. baking soda and regular detergent... It will help remove any staining... Consider getting red underwear for "that time" it helps conceal any "staining" good luck

Washed properly they are completely clean and sanitized.
