i received a call but i did not call them back. can this be a refusal?

I'm assuming you are collecting unemployment. I would think that a job refusal statement would arise from the condition of being actually offered a job. Not calling someone back, although you should have, means that you didn't want to communicate with the employer for some reason. I would ask my job counselor about an appeal, or clarification of what constitutes an actual 'job refusal'.

No. If I didn't answer your question, would your answer have automatically been "no"? On the other hand, it does mean that you might be a slacker for not bothering to answer in a timely manner, and you just got rejected on that one!
