There are numerous ways to designate a border. You could designate it using GPS coordiantes, latitude and longitude, constructing a barrier, it could simply be a mutually agreed boundary, landmarks could be used, physical written discriptions like metes and bounds, etc. However those types of boundaries are typically the ones that create border disputes!

They are decided upon by the two countries if it is a border. If it is a region inside a country it is decided by the country
