"twice removed" means 2 generations beyond being a first cousin.
Suppose your grandmother (let's say your mother's mother) had a 1st cousin, Amy. Your mother would be Amy's 1st cousin once removed and you would be Amy's 1st cousin twice removed-----your child (another generation) would be Amy's first cousin 3 times removed.----and so on down the line.
This is not the same thing as second cousins, third cousins, fourth cousins etc. That's another story.

First cousin twice removed means the grandchild of an aunt or uncle.

Take a pc of paper. On top left put name of your mother and father; on top right put your aunts name (maybe your mothers sister) and her husbands name. Under your parents write your own name. Under your aunts put the name of a daughter(make one up if you need to. OK. You and your aunts daughter are first COUSINS. Now..... Your cousin has a little baby-call her Sue. Sue is your cousin once removed. Sue grows up and has a child-call her Mary. Mary is your cousin 2ce removed. Clear enough?

"Removed" indicates how many generations separate you from a cousin. You and your first cousin are in the same generation so "removed" isn't used.

Your mother is one generation older than you so your mother's first cousin is your first cousin once removed.

Your grandmother is two generations older than you so her first cousin is your first cousin twice removed.
