Most runs in a game, both teams: 49. On August 25, 1922 the Chicago Cubs defeated the Philadelphia Phillies by the score of 26 to 23.

In the major leagues, the answer is indeed 49, when the Cubs beat the Phillies in Chicago, 26-23, on 25-Aug-1922. Box, so you'll believe it: http://www.retrosheet.org/boxesetc/1922/B08250CHN1922.htm

In professional baseball, I'm not sure of the record, but there was a Texas League game in 1902, where the Corsicana Oil Citys beat the Texarkana Casketmakers by a final of 51-3. Corsicana C *** Clarke hit eight home runs in eight at-bats, which was a pretty good day at the plate. Due to local blue laws, the game was played in a neighboring community stadium better designed for football, which had a right field wall estimated at 210 feet (and possibly shorter; the entire game remains a matter of dispute, likely never to be resolved). At a guess, that total of 54 runs probably hasn't been outdone.
