Prepartion:
Firstly, prepare a reading suitable for the child in question.
Then read over the piece yourself in preparation for the lesson picking out the new/difficult words.
Write these new words on flash cards.
Lesson:
Introduce the lesson positvely.
First you read through the lesson SLOWLY
Explain, refering to whole sentences, the new words.
Read the piece with the student.
Now allow the student to read independently.
Repeat this three to four times until the child shows fluidity and comprehension!
Consolidation:
Play a game with the child using the flash cards with the new words on them!
Go through the meaning of the new words on flash cards with the child.
Get him/her to put them into sentences.
Now hide all words.
Teacher picks a word in his head. He gives child clues to this word and child must now quess the word.
eg. word is 'house'
clue: a family live here...what is it?
Another great help would be ''readers' theatre'' , i'm not sure whether it would work with only you and one pupil, but look it up and see if it would be advantageous to you!

I am tutoring a 5th grader right now.
First and foremost, we are getting his summer reading projects done before August!
We also read aloud together and after each page, I ask him literal questions just to check his understanding.
Once we finish a book, he reviews it and says what he likes and did not like. He also makes connections to the book with his own life.
Finally, I purchased a Language Arts workbook at Borders. I usually have him read a short story (2 pages) and then we work on the questions and activities together.
I know this may seem a lot, but we just figure out what we want to work on each week and go from there.

I shouldn't assume, but most of the kids I tutor are having significant issues.
There is a series of workbooks called "The Daily News", they are low readability, high interest. That may be a great place to begin your tutoring. The articles are short and ask comprehension questions. You can incorporate some writing and research into the articles for some added spice. (Not that students think writing is added spice)
Another thing you could try, which a lot of my 7th and 8th grade students that struggle with reading love, is to use Zoobooks or Kids Discover magazine. Each magazine cover one topic. You could develop some comprehension activities from the magazine.

Here's a site with some things on it...not sure how good it is.
Do a search under Reading Lessons Grade 4 (or 5) and see what you come up with.
Or try Reading worksheets, etc.
If you go to the department stores or Chapters a lot of them have books with lessons in there for about $5 or so to help kids better their skills. You might find something in there too.

What exactly do they need help with?
If it's actually making sense of the letters and sounds, they will need a quick remedy for whole word learning that actually lets them access the alphabet code.
Many children can't blend whole words and use a whole range of other strategies that just takes attention away from just absorbing the words and their content.
Comprehension is easiest when the words are read wth as much ease as possible - ensure the child can actually decode what is on the page effortlessly.

It all depends upon what reading level thekid is on. You've gotta asses the kid before you can tutor him along.
I'd start w/ some simpler ones, throw in some Harry Potter ones (like when they discovered theWeasley's Joke Shop or Harry's first time to fly a broom), some Amelia Bedelia, some fact-filled paragraphs from encylopedias or biographies, and maybe even exerpts from classics like Jane Eyre or Hemingway or something.
