The only reason I can think of is that there's something wrong with their head.
But many people never feel guilty because they lack conscience or just have learned how to cope with any situation.
Good for them I must say, Conscience can eat you alive.

Because they have a mental disorder called Antisocial Personality Disorder (or they have traits of the disorder).
It's the basic criminal type, people with the disorder are also called Sociopaths.
In childhood, it's called Conduct Disorder. A milder childhood variation is called Oppositional Defiant Disorder.

They could be psycopaths.
They know the difference between right and wrong and they choose wrong. They are cold and selfish and only consider their own interests.
