Are you talking about knitting something and having a design on it? You can use different colored yarn to "draw" your design in stitches, you can embroider the design on the finished garment, or you can form the design by knitting it in purl stitches if the right side of the piece is knit or knit stitches if you want it to show up on a purl stitch background.

If you are talking about making your own patterns, use graph paper. Each line across would be a row and each line up and down would be a stitch. You could do different colors for knit and purl and for things like cables.
