Nmito is correct.
But you might want to maybe use concrete sealer for a temporary fix.
Try to find out where on the outside it is coming from and maybe redirect the flow of water away from the home.

Water is getting under your slab somehow, it might be a drainage problem which can be repaired about the easiest. Hopefully you don't have a busted pipe, that would involve busting the slab out and repairing it. Been there done that it's messy and costly. You can check the water pressure on your house by looking at the dial on the meter over a period of a couple of hours with absolutely no water use to see if you have a water leak, if it moves over that period of time you have water still moving through your pipes somewhere. The next step is a plumber if that is the case. Good luck , hope it's only a water runoff problem, don't seal the floor until the problem has been identified.

Depends on where the crack is. More info would help.

Generally, remove any places water will stand against the house. Re grade so the water flows away from the house.

If you have water coming off the roof, and flowing down the side, install gutters to re-direct the water away from the house.

If you have water wicking between the slab and wall, you can use a combination of metal flashing and sealer to keep water from entering the house.

If the puddle is below a window, you may have a bad sill. If it looks OK generally, use caulking to seal around the window sill. If it looks old, and crumble, replace the sill.

If you have cracks around teh frame of the window, use an appropriate filler to close the cracks, or replace damaged areas.
