Black holes are real; wormholes are hypothetical.
A black hole is a huge mass with enough gravity to collapse itself into a singularity.
A wormhole (if one existed) technically wouldn't even need "gravity" to function. It would require various forms of exotic matter that have been theorized but never observed.
Wormholes are figments of wishful thinking and science fiction.

First, there are no known wormholes, artificial or natural, in this universe. If there were any wormholes, a person would most likely see the other side of what the wormhole links to (check wikipedia for more details on the mechanics of a wormhole). However, if a person were to look into a black hole, the person would only see pure darkness (hence the name black hole) due to the immense gravity of such objects. So, to distinguish between a black hole and a wormhole depends mainly on whether or not either are emitting light.

A black hole that can be travelled into without getting dissipated in knows as a worm hole. However, the physics of that happening look remote at this moment. Unless some way is found Worm holes are only fictional.

Black holes actually exist, or at least we have very good observational evidence that they do...along with very well thought out and supported theories.
Worm holes simply do not exist...they are a product of science fiction and theoretical astrophysicists that have way too much time on their hands. Even their math has to go into the realm of what is currently considered impossible, even by their own admission.

Seriously, i firmly think you have missed some really important points somewhere along the line here. You
ask how you can tell one from the other, right? Well
a black hole is invisible - can't see it. So, if you can
see it - It obviously isn't a black hole. Black holes
suck in light and none escapes, so they cannot be
seen.

I'm not really much of a gardener, but I would suggest that wormholes have been made by worms, and black holes have had insecticide put down them. (Probably made by termites)

If it exists, it's a black hole.
If it's a sci-fi construct based loosely on studies by Schwartzchild and Einstein, but never observed in the known universe, it's a wormhole.

A wormhole transports you from one place to another ( probably light years away ) in seconds. A black hole crushes you down into the singularity ( basically your crushed to death then compacted till your smaller than an atom ).

If you go into a worm hole, you come out somewhere
If you go into a black hole, as far as we know, you dissipate and cease to be.
