Office Max, or Office Depot, Staples. They all sell blank leases appropriate for the state they are in.

There are many generic lease forms out there. Use caution, since some of them do not include language required for your particular state, and, as well, some include language NOT appropriate for your state.

You are advised to find one specifically designed for the state in which you live.

As far as protecting yourself from renters damaging your house, statutes cover that without reference needed in a lease. However, they may still damage it, leaving you to collect from them in a court of law for the damages.

You are best off to properly qualify the tenant via previous rental history, credit checks, and so forth to minimize the chances of damage.

There are tons of them online. Just use the online searches and find leases that will be a template for designing your own.

They sell the fill in the blank type leases at Staples. Or you can get free copies at your county court house library.

Actually, if you go to your local office supply store, they normally have a place where they have generic contracts and rental agreements that are legal for your state. Be careful about forms on line. many places post forms there that are very generic, but may not have some important parts that are not good for your state.
