You can paint over it with gesso for oil paint. But if the original painting is heavily textured, you won't be able to cover that. It's cheaper and easier to just buy a new canvas. Donate the old painting to a charity if you don't like it.

You can't remove oil paint from a canvas, but there is a specific medium to cover it so you can reuse the canvas. Any art supply store carries it.

I do not believe you can. The texture of the canvas will hold the paint tightly to it. Most artists paint over old works.
