I have, I probably shouldn't have...but I did. It was more of a sympathetic apology rather than a, "I'm the cause of this" apology.

I'm adding this three days later: I'm sorry mister but you have to get back in my life ASAP, I miss you.

Yes a time or two. (Killed me to do such a thing but all is OK now.) Wasn't that bad, I just don't make it a rule of thumb to kiss @ss ;-)

Probably, although i don't specifically remember, but i do say i'm sorry for things like others losses, illnesses or misfortunes. You know: 'I'm sorry your dad died." or, "I'm sorry you lost your job." Stuff like that.
