i work 12-10 , 4 days a week and got into the habit of staying up late, and sleeping till i go to work,, now it seems like all i do is sleep and work, whats the best way to break out of this pattern so i can sleep at a decent time and wake up refreshed to get my active life back??

Those hours are hard, because once you go home the day is over. And when you wake up I am sure you only have a few hours for you to do the things you need to do, plus get ready for work.
But you do have 3 days off. Which is the days that you need to find things that you like to do and enjoy.

As far as winding down after work. I assume you eat dinner when you get off work at 10, unless you consider your lunch time your dinner. Once you get home, take a nice shower or bath and try your best to get to bed. That way you will wake up earlier and have more time in the morning for yourself.
