It sounds like you need to push for another exam by a different doctor. If your first doctor was not a gynecologist, get a referral to a gynecologist. If the first doctor was a gynecologist, get a referral to a different gynecologist.
It will help if you write down your problems on a piece of paper, and keep the paper in front of you when you talk to the doctor and the doctor's staff to remind you of what the problems are. You need to stick to the subject and keep reminding them why you are there. Don't let them dismiss you without responding. They need to tell you what is causing your problems and how they can be treated, and what to do if the treatment does not help.
It is hard to tell from the amount of detail you gave, but one possible diagnosis is prolapsed uterus. I'm putting a link in sources that gives more information.
Another possible diagnosis is related to menopause. In menopause the ovaries stop producing estrogen, and that leads to withering of the vagina and dryness. It might be that hormone replacement therapy and hormone creams could help.
There are other sex lubricants on the market that do a much better job than KY Jelly, and it might help to try some of them.
Of course your spinal cord injury and surgery could be a factor. You should ask your gynecologist about a referral to a specialty neurologist or a specialist in physical medicine & rehabilitation (PM&R). PM&R is also called "physiatry" which looks like the word psychiatry (mental health) but they are entirely different fields.)

if you are past the menopause you may have a problem due to low estrogen resulting in the dryness. you can get hormone cream for the vagina but there is also an over the counter product called replens for women who cant or dont want to use hormones. after menopause the tissue of the vagina becomes very thin and fragile and dry. the vagina can shrink and sex can become painful. both estrogen cream and replens can help this. as far as dropping, that would need to be corrected with surgery, but you would first want the tissue to be in good condition, probably would want to use the estrogen cream for the best effect. And it is a VAGINA.

Oh sweetheart, there is a condition where the vagina becomes very tight. However i think it is along the lines of a phobia. You tense up at the thought of sex, even unconsciously, and yes it's painful.
Even though you are probably sick of doctors, the best person to ask would be a gyno.

Have surgery to put things back in place. You probably have a prolapsed bladder. If Dr didn't explain or help go to another GYN.

You really need to see a gyno about this. It may be uterine prolapse, which can be treated. It may be lots of things...but I'm pretty sure someone in the medical field could help you.
