like on the front its the cable, but then on the back of the work do you twist it again like on the front or do you just knit and purl the back row and then just keep on cabling on the front?

In many patterns, you knit as the stitch as it appears. By the way, knitting cables and other patterns is much easier in the round.
You might want to look into learning circular knitting.

Your pattern will tell you what to do.
Mostly it is the same as the stitch you see.
If the stitch is knit, you knit it. If it is purl, purl it will be.
There are few patterns that needs to be changed each row.
Many patterns will even only be worked each fourth row, (each second row on the good side.)

Or were you talking about what to do to the back of your sweater, many are just plain, but you can do patterns there too.

Usually a cable pattern will give you clear instructions for each row of the pattern. For example, look at the Irish Hiking Scarf free pattern that you will be able to find making a simple Google search. The pattern is very clearly explained and you will be able to figure out what you should do on the right side and the back side of your work if you start from scratch. I hope that this will be useful for you.
