I have but I always go to a public place like a bar/restaurant and I bring one of my girlfriends with me who will sit at the opposite end of the bar - if the guy is okay - I give her the thumbs up on the sly - if not, I excuse myself to the ladies - text her from my cell phone to call me and pretend to be a family member with an emergency call. Mostly - I have been sneaking off to the ladies - just bad luck lately with these men.

I have, and we're still together. One of my relatives knew one of my relatives though. But I still had never seen him in my life. I would take any opportunity, you never know what might happen.

NO. If I've never met him, for all I know, he could be married, a rapist, a serial killer, or any other kind of creep. I'd rather get to know someone and be friends before I start that kind of relationship.

Only if I knew and trusted the person who set it up.

No, because if the person is ugly, I have to think up a lie of why I have to leave. Awkward.
