Square feet means you're looking for the area. Area = length x width
and Cubic feet is volume. Vol = length x width x height
length, width and height must all be expressed in feet

Length times width gives you square feet. 1250 is the simple answer. But if you are making an order for pouring concrete, you need the cubic feet involved. So you need the depth, too. If it is 4" that is 1/3 of a foot and you have 416 cubic feet. However, concrete is ordered in cubic yards, so divide that by 27 (3'x3'x3') and you get 15.4.

Yes. You'll need forms. And good prop supports to hold that much weight. You'll also need a vibrator to get bubbles off top of your angle. If I was you I'd call the concrete place and ask for the number for a experienced concrete laborer. He'll have the tools and the know how.
