Here goes... I always get a thumbs-down when I answer this question, but The Cure (with Brad Renfro) is no doubt the saddest movie I've ever seen. I saw it again Tuesday on Encore and bawled my eyes out.

Ahh yes, Ladder 49 has made me cry all 4 times I've watched it.
Hah I'm a sap.
I wouldn't say it's the saddest I've ever seen, but it's definitely a good one.

Depends on what kind of movies you like. Took a quick check around the internet for a list, but this one covers most of the bases for a wide range of people...

--------------------------------
Eternal Sunshine Of The Spotless Mind
Requiem for a Dream
Old Yeller
Gladiator
Forrest Gump
One Flew Over the Cuckoo's Nest
Edward Scissorhands

A walk to remember,Ghost,Steel Magnolias. This might sound weird but I think that Cast Away is so sad,the end really hurts my feelings. The saddest of all though is probably Beaches.
Edit:I just remembered Swing Kids.

Rent or buy "Central Station," a Brazillian film that came out about eight years ago.

The ending is quite hopeful, yet you'll cry like a baby. Oy, I'm tearing up just thinking about it.
