See a hand surgery specialist. Most of these are orthopedic surgeons who have further specialized in hand conditions, but plastic surgeons and general surgeons also sub-specialize in hand work.

Ask your doctor for a referral. When I looked on the site for the American Association of Hand Surgeons, there was a disclaimer that not all members credentials have been thoroughly scrutinized.
