And where are your four children and their two tagalong friends while this precious scenario is acted out?
Personally, I don't "do" fast food and rarely have unless in transit or some such very rare event. However, it seems to me that one parent should be settling the kids into their seats and keeping them occupied while the other stands in line, already knowing pretty well what the family wants.
Or the family takes a seat and reads and makes out the menu and one parent goes stand in line. IF CIRCUMSTANCES permit, the parent with the kids, or an older child can tend to drinks and all the usual little things, straws, napkins, etc. It little matters which parent does which, they are a team remember?
I cannot BELIEVE that someone has actually worked out this restaurant scenario so precisely so that there is actually a CODE for eating at a fast food joint! I will be honest, sounds so cut n dried! Is there also a set of rules regarding how many straws one should use for this drink or that?

Wow! I had no idea that there was fast food etiquette! I did not even know it, but yes I do that! Even though I hate fast food, but whenever we have gone to a fast food restaurant in the past. I would always go get the napkins, straws, ketchup, and so forth and go back to line to wait for the rest of the order. Most of the time though the drinks were given to us. So I never had the opportunity to go get the drinks. Most people this day in age would find following this type of etiquette to be demeaning to women. However, if women would actually take a minute to think about it, the man is the one who has to juggle all of the food and drinks. All she has to do is help find a good seat! I much prefer it that way!

When we eat fast food it's at the ballgames. Whoever is going to the stand brings back all required condiments and silverware.
We do not take the children to "fast food" to eat. If we eat out it's at a restaurant.
Just out of curiosity: you said, "Proper fast food etiquette for couples or families states that ...."
Exactly what etiquette source is that?
Also: What exacly drives you to incapacitate the male species?
According to you after she gets his drink for him she has to cut up his food for him and put it in his mouth for him. If he uses the toilet she has to go in and clean up for him.
Exactly what about that is attractive about a man?
Personally, I find this completely emasculating behavior, and a total sexual turn off, If I may be so blunt.

I mean, in some aspect you are right, to a point and to an extent. But you seem to take that WAY too far! I mean, listen to yourself, do hear how you treat females??? Do you understand why females don't like the way you act, talk, and think??? I mean, are you ever going to make an effort to change your ways? For the better? Less harsh maybe??? Actually, my man can't take me out to eat. He has no driver's license. Is there a code on that?
And I do all that stuff and he gets the seats. Are we backwards or something? I'm the one who works too. I just don't even go into a fast food with him now.

Sadly, I used too...didn't know it was a code though just seemed like the nice polite thing to do. What if I am buying, does that mean my man has to get the straws and stuff? Is it wrong of me to slip my man the money so it looks like he is buying in order to protect his delicate manliness?

I don't think the words "fast food" and "etiquette" go in the same sentence right, if you get what I mean.
I've been to fast food places with a guy, and that rule was; Get food, male sits and scoffs his face, female sits politely gagging, male gets up and leaves, female follows on, trying not to vom at the sound of him eating so loud.
Do you see the pattern here?

I have done this before. I'll get napkins, ketchup, etc. while waiting in line as well. I do this as a courtesy. But had I known this was mandated in stone, I would never do it. If it's something expected of my, I rebel. If it's my choice, I'll happily do it!
