he did in The Order of Phoenix against Bellatrix after she had killed Sirius. it didn't work because, as Bellatrix said, he did not want it so badly as for the curse to actually work; he wasn't mean enough!

he used "Crucio" against Bellatrix Lestrange in Order of the Phoenix after she killed Sirius but even then his heart wasn't in it so it didn't "work" properly. In Half Blood Prince when Snape was fleeing the school he tried again but only got to say "Cruc-" before one of the other Death Eaters hit him with a spell and that sent him to the ground (and then he lost his wand in the dark).

Yeah i think he did Cursio to Beatrix in Harry Potter Order of the Pheonix, but he didn't have the guts to kill her.

YES! He did in Deathly Hallows. He used imperio on the head goblin at Gringotts.

Yes in the order of the phoenix he uses excrucio curse againbst the woman who screwed up nevels mam and dad

i really dont think he did...isnt harry potter supposed to be good and fight evil....not use the unforgivable curses...anyway
