Casey Stangel..oops, Stengel has the "stangest" name in sports history!
Why sports and not baseball history? Baseball, I'd go with Urban Shocker.

For baseball, I would have to say the former double play combo of Mark Grudzielanek to Doug Mientkiewicz is a good one.
Football... it has to be Dick Butkis, no question about it.
Hockey... well... all of those names are strange, but Miroslav Satan is a good one. or Steve Yzerman
Basketball, Hakeem Olajuwan (probably not the correct spelling), or Manute Bol.

Obviously that would be backup infielder Doug Strange.
And of course, grossest last name would belong to Kevin and Greg Gross, and sickest last name would go to Ed Sicking.

Doug "Eyechart" Gwosdz.
Pretty much the only player I have to look up to make sure I spell it correctly.
For bonus fun, try to spell it phonetically so people know how to pronounce it.
