There are two "Sick" a documentary...very disturbing and thought provoking. It's about a very intelligent and creative man who is dying of cystic fibrosis..he and his woman get very involved in sado-masochism. Not for the squeamish but it will cause deep empathy as well as total disgust.
Breaking The Waves with Emily Watson. It is my favorite all-time movie. EVERYTHING about it is excellently done and also very heart-breaking and thought-provoking. Both are quite "dark" if you like that genre.

Thats hard since you didn't give a genre or your location. My favorite 3 movies that weren't "out there" publicity wise are:
Love Song for Bobby Long (a great moving about the deep south and some really unique relationships starring John Travolta in the best role he's every played ... I'm not a real fan of his movies, but this one is definitely worth a view)
Finding Forrester - I think this one may have gained some popularity over time ... but it was a sleeper for a long time. Stars Sean Connery
Normal - the story about a transexual who comes out in later life in a very Bible Belt community. Its sensitive, informative and touching. Some of it is a bit over the top in that it could never happen they way they present in the real world ... but the basic story is so true.

I'm going to have to say Oldboy. While it won a few awards ( including the Cannes Special Jury Prize several years ago), I don't think it is that well known with casual film watchers. Oldboy is a South Korean revenge flick that blends both style and substance. Its hard to talk about without spoiling anything, so I'll just give you the basic info. A man is kidnapped and held in a room for 12-13 years. While in the room he sees a newscast in which he is blamed for the murder of his wife and daughter. He trains and keeps himself physically fit while stuck in the room. One day, he is released on top of a building, and the film truly begins. Anything beyond that would spoil the story. It is a prime example of the awesome films you can only get from non-Hollywood machines.
It reminds me a lot of a combo between Quentin Tarantino and Christopher Nolan

It's written by the same guy who wrote the script for 'Being John Malkovitch' and 'Eternal Sunshine of the Spotless Mind'.
It's all about the difference between humans and apes. It tells the story of a man who was brought up in the wild by a lunatic who taught him that he was an ape, a woman who is very hairy, and a psychologist who experiments on training animals. It's really funny and touching and the ending is an incredible twist.

If you like period pieces Pope Joan(2009) is quite good and the documentary "The Bridge" about the Golden Gate Bridge and how it is a famously popular place to commit suicide. Its kind of sad though. But very good.

Well, the first half of the movie is pretty good. It is pretty suspenseful. The second half gets a bit goofy but there is a decent suspenseful ending.
Damon S is right. "Boondock Saints" is one of those movies that nobody knows about but was actually pretty good.
