It was a combo. If you remember from Revenge of the Sith *when Obi Wan turned Anakin into the human potholder >_<*..the only way Anakin was still alive was through the breathing apparadus that was attatched through the body armor and helmet..so when Old Palpy *my loving nickname for Palpatine* let loose with the bolts, Vader got fried as well..so it was a combo of things.
Also, remember, Vader DID have his helmet off during "Empire Strikes Back"

Vader's suit is basically a life support system that controls all his bodily functions including breathing. The bolts of lightening fried that system to the point of no return. Of course, some may say he died of a broken heart as well or the evil of the sith within him destroyed him. You make the call.

It was still the lightning bolts. The whole point is that Luke has surpassed Darth Vader... not to mention the fact that he is a lot younger and more human. Don't forget that Vader was kept alive by the mechanisms of his suit. All that electricity could not have been good for his breathing apparatus and life-support systems.

The Emporor was the one who killed Darth Vader, when he was lying down, he was already slowly dying and couldn't breath. So when Luke took off his helmet he was about to die, then died. So it wasn't Luke's fault.

What actually killed Darth Vader?
the explosion of the death star he was dying do the failing of the suit but mostly because what kept him alive was his hatred for everything and when he saved his son the hatred left him and his soul could pass on.

darth was wounded by the electric, ( his body is mostly a machine, whereas luke was not)
then being unable to get off in time before the death star exploded, he wanted to see his son with his own eyes,
so he took off his helmet ( and associated breathing systems) to see him
so, depending on whether he actually died of asphyxiation, or as a result of being caught in an explosion anyones guess.

Think about what Obi Wan said when he first talks to Luke about Vader; "He's more machine than man now, twisted and evil". What happens to a machine when you run several hundred thousand volts through it's circutry? Now apply that to a cyborge and you've got what happened to Vader.
