Isn't it Denethor? I'm sure they removed him from the theatrical version and so it only ended up in the extended DVD.

I think you must have made a mistake - you say they're on the 3rd disc of the 'normal version' - surely the normal version only has one disc, maybe two at the most - its the extended editions that have 4 discs.

there is definetly a guy whose head pops up as if on the way to the toilet on my DVD copy which occurs in both the above - I can't make out if it's actually the same guy / girl though and it definetly occurs at different points in both films.
