You must appear somewhere. Court or a lawyer's office for deposition

Plead not guilty, usually at arraignment

A meeting before the trial. Two lawyers looking to make a deal or meet with the judge to iron out some points of law.

Waive "speedy trial" requirements or waive a time limit or date previously agreed upon.

Why are you requesting a new lawyer. The other side smells something fishy.

Notice of Appearance means that one would be noticed to appear before the court and account for what the (fraudulent - in most cases) cause of action is for ....

Plea of Not Guilty means nothing - one is guilty until proven innocent - which is not very likely when the cause of action is stated and no defense is allowed, no questioning of the accusers is allowed - no jury is allowed or no Offering of Testimony/Evidence is allowed unless the accusers ALLOW IT ...

Pre-Trial Conference Request is just becaue they really have no case and want to have one admit guilt so that a case or cause may be established ...

Time waver - is for one who is not guilty - or time limitations HAVE EXPIRED - CONSENTS TO THE IDIOTS TO HAVE THE TIME WAIVER ALLOWED SO THEY MAY BE PROSECUTED TO THE FULLEST ... instead of standing on their rights because they do not know what their rights are ...

Demand For Discover Substitutin of Counsel Entry is simply a nice way to say that whatever your evidence you want to bring forth will not be allowed - as a substitution of counsel has been made that may not even know of what you request - but the prior party full well knew what it was - but ...

they Substituted the Counsel so that you can not obtain the information ... nice legal trickery that Public Information/ Open Records Act could help in the long run ... get the old party to answer within 10 days or report then to the Attorney General of your State ... then watch them all squirm - and do not take ... oh- there is no information available pursuant to the request ...

Tell them that the information was available - AND DOES NOT NEED TO BE CREATED - IT IS BONA FIDE FACT THAT THE PLAINTIFFS USE AGAINST YOU - AND IT MUST THEREFORE EXIST ...

Then go to war in a peaceful way ... the pen is mightier than the sword - and it really hurts when truth is displayed by it ...

I'm sure of all of your questions but I believe that:

Notice of Appearance means you have to appear in court
Plea of NG means you plead No Contest
Pre-Trial Conference is a plea bargain
Time Waver means you can call for a continuance giving you more time to state your case.
Demand for discovery means the plaintiff demands that the prosecution has the same evidence as the defendant.
