It spreads and all those people/cells that fall to it are victims of the disease. I think what Paul Simon meant was if we let people continue to liive in silence, they never fight back for what they believe in, and a society where no one is willing tio stand up will form, and that is like a cancer, it kills the society.

Many cancers grow without being detected.
Silence becomes the norm when people stop communicating. Many times, the silence is not recognized until it has been there for some time.

Well, in the song, I think it's referring to how if you do nothing about the evils in society, the poor and remain complacent(IE silent) it's like a tumor and will eventually kill like a cancer. Rush also took this concept later and used it in a different perspective (middle class) in their song Subdivisions.
