I nervier liked The Royal Flush Gang. A little too gimmicky for my taste.
If you mean worst as in most evil, then it's The Joker hands-down.

Im pretty sure you would have to ask a Comic Book fan about this since most Batman fans noawdays only know villains from video games and movies and in the games and movies they only use the good villains, the worst villains are in comics.

To my mind, there are a lot of second rate Batman vilains. The Mad Hatter is a good example. He is nuts and he has this schtick: he likes to steal hats. Doe sthat make him a super villain, worthy of squaring off against the World's Greatest Detective?
Okay, but my personal favorite: Doctor No-Face. (Real old, early 60's.) A criminal with identifiable facial scars secretly visited a plastic surgeon who was experimenting with a skin-regeneration ray. The doctor protested that it wasn't ready to be used on humans yet. The criminal tried it and it erased his face! There was nothing but bare skin there. (transparent membrane over the eyes, no nose, mouth and invisibly small slit, no ears.)
The criminal had remarkable presence of mind. They kidnapped the doctor and he impersonated him. A failed 'demonstration' and the public thought that the doctor was the one who had lost his face. He 'went insane' and went on a destructive spree, destroying masks, valuable portraits, the 'faces' of antique clocks, the 'facets' of rare gems.
(He actually stole the valuables and substituted cheap imitations for them, which he destroyed. ) When batman 'captured ' him, of course the courts would have lenience on him, in view of the terrible disfigurment he suffered, but 'his' face (the real doctor's face!) would be restored through plastic surgery, and he would have the perfect hiding place, and secretly sell the valuables.
Batman checked his fingerprints.
Can you say, What a waste of a great concept...? I knew you could! Can you say, LAME...?
But then, look at how some of those writers kept recycling the Clayface theme, long after it was beat to death. oh, well.

The Joker hands down. If you've never read the comics then you wouldn't know he is so much more than what the 1960's TV series portrayed him as.
