No. Women are born with all the eggs they'll ever have, but men produce sperm throughout their lives. The quantity produced reduces over time, but in most men production continues until they die. Men of 80 have fathered children!

Right after you ejaculate, it is possible to lose all (almost) of your sperm... But no, in general, puberty and masturbation cannot cause you to lose the ability to produce sperm. It could be for other reasons, but those two you listed should increase your sperm, actually.
You should seriously consider seeing a doctor. What you describe is extremely rare at your age.

No way! Sperm builds itself back up every day, and even within hours if you're young. Chances are your sperm will be good until you're a very old man, and before it goes bad you'll probably be too old to care! So don't worry. It's actually healthy in many ways.

No, that's just a tale some adults tell to keep the youth from being 'naughty'
Sperm are continually reproduced, just like all the cells in the body. Unless your body is physically incapable, due to an infection or other happening, you should be perfectly fine.
If you're worried, however, you could always check with a doctor and see what he or she has to say on the matter.

No, men are continually making millions of sperm cells until they die. Unlike women (who are born with all the eggs they will ever have) men are constantly producing new sperm.
