i had the same problem with applicator tampons, try getting some non-applicator types (o.b. is my fav, and they have procomfort now to help with insertion)
your body is different from other women's so the curve of your vagina prolly isnt suited for a straight applicator (neither is mine)
try the non-applicator kind, or get some INSTEAD cups or the DivaCup.

You may be having problems if you are not using the right absorbancy for your flow. It might be helpful to only use the light flow tampons at the beginning and ending of your periods until you begin to get more comfortable, and then move to using tampons for your whole cycle. Once you do start using tampons regularly, they must be changed every 4-6 hours. Always remember to remove your tampon, and never try to use more than one at a time. If you have a heavy flow, you should use a combination of a tampon and a pad.

You need to relax when you're putting a tampon in. If you're not, the muscles in your vagina tighten up and it makes it difficult to put a tampon in. If you don't put it in the right-ish spot [there isn't really a right spot for it, but there is a spot where you can't feel it after it's in.. supposedly] it will feel like it's going to fall out, but it most likely won't.
My suggestion is to relax and if you still have trouble putting it in, use a smaller size until you get used to it.
Also check out www.divacup.com and read up on the product. You'll save more money than using tampons and pads.

A tampon only hurts if it isn't inserted correctly. Keep trying. You'll get it eventually.

Are you using the right level of absorption? If you're using a super and you only need a regular, it will hurt going in and going out. Just keep trying. You'll learn your body and you'll figure it out.

You have it inserted incorrectly if it hurts. Once you insert it correctly, you should not be able to feel it there.
