My wife worked at the Desert Inn before it went down, I worked at the Hilton Hotel part time. Yes, I knew Paris Hilton many years ago when she was a teen age snot.
The maids decide this, when the sheets, pillow cases, towels, the linens in general, when they get bad the maids toss them.
It varies so much according to there use, it's hard to say. Some hotels are garbage dumps and will change only when really ragged, some change to make an impression.
We stayed at the Golden Nugget years ago, the sheets had so many stains from blood, to bodily fluids we refused to sleep on them, when we complained, they brought new sheets that were just as bad, although we complained again, to no avail, we slept on the covers, we couldn't see the stains on these, the colors.
It was a real dump, it didn't change until Steve Winn owned it but, only for a while.

I work for a Hotel, towels, sheets are washed everyday. Every 2 days I take the blankets and spread off to be cleaned. If the sheets, towels, etc.. are stained we don't use.. One other Hotel I worked for it the towels or sheets gets a ink stain in they are discarded immediately. It's sad to say but other Hotel chains will continue to use the stain sheets and towels.

all i can say is YUCK!!!! You wouldnt catch me sleeping on hotel sheets at all. I bring my own!
