it's basically that by revealing something (usually secret) it creates a host of problems which spiral out of control. eg. revealing you had an affair with your partners brother destroys your relationship and also your partners brothers, makes both sets of children parentless etc

An open can of worms is a whole bunch of wriggling things going everywhere at once.
If you ask a question or make a statement that has a whole bunch of unintended consequences, you are opening up a can of worms.
Example: "Did you know you're almost as good in bed as your sister?"
