A question about formmail?

I am trying to create a website...which is all new to me...anyway, I want to have a contact page where customers can input their name, email, phone # and a breif message...and I want to have a send button that sends all that data to my yahoo email address. I am confused beyond belief...could someone please tell me how to type out the code in HTML? Thanks in advance
