In most cases, it's real. Digitally editing a porn movie in the same way that Hollywood flicks are edited would take a lot of money and porn movies usually have a smaller budget.
There are, however, techniques to enhance the way it looks on screen. Most men have their pubic hair trimmed or shaved off completely. Natural hair can cover up to 1 inch of a man's penis. So trimming that off adds an extra inch (visually, at least).
Also, men in porn are in good shape and slim. This helps more of their penis show. If you have extra weight in your stomach (as many of us "regular" guys do), that covers up part of your penis with body fat. So again, a man can look larger than your average guy.

Sometimes yes it can be fake, but there are also many techniques they use to make them appear much much larger than they actually. They edit tapes as well to make it seem they have been going for the entire tape but really they have had a few breaks.
