Having your body cut up everywhere by a razor blade. Then after that, you're thrown onto a bed of sand. After that, you're thrown in a large vat of lemon juice. Not only will that hurt a terrible deal of pain, you'll drown there. Finally, when you're dead your body is removed, cut up and put on a grill with BBQ sauce.

Burned to death. I have a really big fear of fire because I don't want to die that way. I think that is probably one of the worst and most horrific ways to die.

Being raped and tortured to death, like in the Quentin Tarentino-ish movies. A second worst is being buried alive.
Compared to that, meeting death by suicide would be like meeting a loved one.

being gang raped then having your breasts hacked off and a bottle pushed up inside you then having it shatered by having your stomach stamped on, then left to die. i think that would be a bit horrid. or getting run over.
