I know you mean profesional, but apparently it happened during a high school baseball game. In 1982, Hatboro-Horsham H.S. (PA) against Quakertown H.S. Left fielder Steve Bradley was the third batter. He K'd. Team batted around, he grounded out to second, driving in a run, the team batted around again, and Bradley flew out to center.

Are you talking about a player at the plate, or a player in the field?

In the field, it's happened a number of times. A pitcher striking out the side, an unassisted triple play, etc. I'm sure there have been a few times where there were, say, three fly balls to the center fielder or something like that.

At the plate, no, not in the majors. There have been a few guys who have had three plate appearances in an inning, but all of them reached base at least once.

As a batter... I do not think it has ever happened. I know batters have made two outs in an inning. In the field, on defense... it is called an "unassisted triple play."

It has happened twice in the last few years and most often involves the second baseman or the shortstop, as they can possibly catch the ball, tag second base and tag a runner out ALL in the same play.

there have been a grand total of 13 in MLB history. The most recent was shortstop Troy Tulowitski for the Colorado Rockies earlier THIS season (2007)

Oh... and the silver and golden sombrero refers to striking out (as a batter) in a game.

There have been 13 unassisted triple plays in MLB since 1900 including Bill Wambsganss, October 10, 1920 in the World Series. The most recent was Troy Tulowitzki, April 29, 2007.
And the sombrero refers to the number of times one player strikes out in a game. The golden sombrero means that you struck out 4 times in one game.

On offense -- no. Not in the majors.

On defense, sure. Typically it's the first baseman, or catcher if the pitcher has his strikeout mojo workin'.
