What happens in a grand slam final if a player has to concede due to injury?
Do they re-arrange the match or hand the cup to the opponent?

Unfortunately they lose the match. Part of tennis is fitness of the players. You cannot punish one player for another ones misfortune.

The cup is "handed over" to the opponent - not a good way to win but there's not much that he/she can do other than start a collection of grand slam trophies OR add the trophy to the collection of grand slam trophies ;-)

The trophy would go to their final round opponent. Obviously, it would be a lousy way to lose, but I don't think the winner would be that thrilled either.
