First off change the HT leads, that may solve the starting problem, if it doesn't cure the blue smoke as well have the engine pressure checked, as blue smoke is normally a sign of burning oil and you may have a piston seal problem.

burning oil. either through worn piston rings and walls or through seals in the head. get it rebuilt/replaced or find a different car. as for the camshaft, that wouldnt cause it, if it is causing it then your head is jacked up and will need rebuild/replace. which is just as bad. sounds like it either has high mileage, or the engine has been run hard a lot... (being newer like an 03 anyway)
you would be most lucky if its a head gasket, the gasket is cheap but labor will run between 600 and 1000. if its been burning oil for a while and it turns out to be a head gasket, just get the motor replaced.

You may have an oil fouled spark plug. It sounds like you have a bad piston or worn rings. The oil that gets past the piston rings will foul spark plugs (and can show up as a bad spark plug lead on a diagnostic.) Do a wet and dry compression test. If it doesn't pass the compression test you will need an engine rebuild. Going up to a hotter spark plug and running thicker motor oil may work as a temporary fix. but your engine is most likely worn out and will need to be rebuilt soon.
Good luck!

The blue smoke means your engine is burning oil due to wear in the piston rings and cylinder walls. It's time for a new car.

Hi
You could have a worn valve guide which is allowing oil to be sucked past it under load, or you could have worn rings.
You will be able to isolate the problematic cylinder by removing the plugs and checking for oil or soot on the firing ends.
A compression test of each cylinder as you remove each plug will tell you a story.
The cost will depend on the problem but it wont be cheap either way.

blue smoke means it's burning oil, usually thru the rings being worn on the pistons, but can also be a bad valve etc.
Black smoke means it's running too rich (probably a stuck choke)
white smoke means it's either running really lean or a valve stem seal is leaking (that usually only happens when you first start it cold)
As for shakking, sounds like either the oil is fouling your plugs, or you need a tune up. Pull each plug and check it for color and oil, if coated in oil, that's the cylinder with the problem. On plugs, black sooty means running rich(too much fuel) white or light brown is correct, and as I said black and oily, means problems with either that piston or valve.
Good luck

i reckon my personal opinion is that car is burning oil and the camshaft in the engine faulty. Just needs repairing, u dnt need whole new engine as it happened too me before and cost me 250 quid. Find a decent cheap mechanic too fix it.
Take care and dont worry x

I think that it might need an oil and oil filter change. I am not sure how much it will cost depending in where you go.
