I know that most people recomend, even to pregnant women, to gargle with warm water and salt, but I tried recently this recipe and it worked great for my sore throat. Is it the salt and water only that works or the combination of three ingredients?

Honey is an antioxidant, antibacterial and antibiotic, it inhibits the virus that grows in the throat and kills it, the water works as a carrier and the lemon juice contains alpha hydroxides that work as a lymphatic toner for the system. The honey helps the body to expel the mucus and Flem and reduces the swelling. Salt oxygenates the water and corrects the PH (7 perfect-normal) which allow the body to heal the bacteria and become well again.
I love it, it works and I swore by it.

This works as a triad:
1. Salt to cleanse
2. Lemon/acidic to cut thru and strip away mucous
3. Honey to soothe after all the cleansing

yup pretty much, honey and lemon just taste good, and are usually warmed for a sore throat so feel sootheing, but they don't do anything about the cause. Water and salt, actually pull the fluid and swelling from inflammation out of the tissues, there by decreasing pressure in the tissues which causes the pain.

The salt changes the PH in your throat which kills most of the bacteria. The honey and lemon soothe the irritation, diminish the mucus, and add some mild antibacterial properties of their own.

Honey and lemon for taste.
Salt and lemon to change the ph of your mouth to make it inhospitable for the bacteria causing the sore throat.

I wouldn't use honey, that could create more flem, but salt, warm water, and lemon works great!! lemons naturally clear throats, and kill germs, and warm water and salt help heal it... so the mixture is a good idea.
