Would like to know if anyone has every set up loaner program of Ham radio equipment. For Hams in Nursing homes or living at home. Would be nice to see if we had a system set up to keep track of Hams we don't hear on the air. And to assist them to get back on the air. Or to help re locate their station and antennas if needed from let's say the basement to up stairs living area.

Can't help with this q but I would like to make a comment if you don't mind.
I think it would be a wonderful idea if you can set up some equipment for older people.
I don't think the person who said "teach them to go on line instead" fully appreciates what it means to hold an amateur licence,
My husband is a G0 and we go to our local club twice a month. The members are all ages.
The speeds some of them achieve in Morse are incredible and I think as hams age they would rather keep to the traditional versions to communicate than use programmes like Echolink for example.
The only thing I could suggest is you search the web for radio clubs in your area, I know our club gets involved in helping out maybe there is a helpful membership where you live.

Sounds like a great idea to me. Most hams have some kind of equipment, of course, but that doesn't always mean they've kept it in working order, or that it's accessible. Handi-talkies mean that hams have more options as far as having access to the airwaves in nursing homes. Unfortunately, you do have to consider the theft problem in some places, but other than that, it's certainly something to look into.

Why ham radio? Just teach them to go online.
