To make a necklace like this you start with a loop and make Square knots:

When you come to the shells, feed two strings through a hole and let the other two go round the outside. Continue with Square knots.

Where the star is they have done something fancy, like gluing the shells together, or making holes in the end of three shells. You will have to try to get the shells fixed in the star-shape, I see that they did go through the holes several times to fix them.

When you get to the end you put a bead on the string that is just right for the loop you made to start with.
You can make your starting loop a bit bigger by pulling on it, or a bit smaller by pulling on the first knot, but remember that this is not easy, better take the right size of bead.
Here is a link that shows how to start, (it shows a key fob, but for the necklace you can start the same way.)

The necklace cord looks like macrame. I imagine the shells are just strung in at the intervals you'd like them to be. There's a great website for beginners called "The Macrame Site". It offers instructions for the knots and some free patterns.
