Ask if they have a pc fillable form available. Many times they do.

There used to be a program called Omniform that you scanned in the form and it recognized the boxes for you to fill in, then printed back onto your original form.

Also, libraries and most offices have typewriters for such issues. Ask around and I bet you can find one.

If you have a scanner you can scan the form on your pc and type on to it, then print it out. Or see if there is a typewriter you can use at your local library.

the only way you can fill out an actual form is to scan it into your computer with Optical Character Recognition software, then fill in the spaces.

Go to a library... they still have typewriters there and you should be able to use one to fill in that form

You can use any software like MS Word to create the document and convert the font style to Century.
It's like the typewriter font ;-) you may also like to use bold to make it even realistic.
And then just print it out.

Most forms are available on line - I would check the Internet first. Be aware that some are in .pdf format - you will need Acrobat Reader - a free download.
