If I were you I would try to get a letter from your doctor explaining your problem and go to the Court office and ask to see the relevant official. Your Dr can write you a letter that you attach to your Jury Duty form that will excuse you from Jury Duty. They may call you again and you may need to get a note again unless your Dr states in the note that this is permanent and you will never be able to perform Jury Duty.

very personal problem-why not explain your case to parties concerned at your legal services if it is to be very long ( your sickness){ This answer is subject to your seriousness about your sickness-if it's a joke thrown on me for some test then I would advise to carry a" pot" with you not to lose your position in jury system ( so much at heart),sorry my dear no hard feelings against me.Then how is your jury service-do they take sick people there?
