Depends where you are coming from. North American equipment plugs right in.

Depends upon who built the resort or hotel. A few have 220-240 voltage @ 50 hz. but most are North American - 110 -120 @ 60hz. or even a combination. The public grid is North American style.

Personally I travel with electrical items (camera charger, laptop, hairdryer, travel iron, razor etc.) that will run on a range of voltages. This eliminates the need for a heavy transformer. I just need the lighter receptacle adapters. Look for something like AC 100-240, 50-60 Hz. on your electrical appliance.

If you're coming from North America the adapter would be the one with two round pins.

Most hotels in Cuba will have a couple of plugs that are the same Voltage as we have here. Take a converter just to be safe but I have been twice to 2 different hotels and have never needed one.

Yes, you need a converter. When I went, I didn't think to bring one, luckily however, the hotel provided one. Any store that sells travel stuff carries converters, and most of them are universal so that they can be used in more than one country.

If coming from the US you don't need one, if Europe then possibly you will. I would buy one just for travel purposes and keep it in a closet with some travel gear you will always take on trips, it's great to have those things at hand in emergencies sometimes.
